To join Modicare, you need to fulfil the following criteria :

1. You should be above the age of 18 years.
2. You should have a referring agent or the Sponsor with their MCA number.

Once you have the above information, you can proceed with the application.

You can become Modicare member either online or even offline.

Online Method :

Once you click the registration link, Enter the following information :

• Sponsor MCA Number ( Use 51972356 )

After you enter the Sponsor MCA number and proceed to the next step, you will see the website showing the details of your sponsor :

Once you hit, website will ask for following details :

1. Your First Name
2. Your Last Name
3. Date of Birth

Once you move to the next step, you will be asked to provide :

1. Your Addresd
2. Your telephone Number
3. Your Email address
4. PAN Number

You will see the popup to perform KYC. Initiate the KYC (We recommend AADHAAR)

After you complete the previous step, you will be required to provide your bank details :

You will see the summary of your form. Verify the information and click Register.
